What is an SRS?
Registered and regulated by the Department of Health as low care, Eliza Park Supported Residential Service provides its clients with personal support which includes assistance with personal hygiene, providing meals, medication supervision and links to social and medical services. The client group is predominantly a marginalised and socially disadvantaged group, many with backgrounds including homelessness, disability and mental health needs.A Manager, Personal Support Coordinator, Personal care staff (P.C.As), Cleaners and Cooks are all employed to be on site to support our clients.

Referring clients to Eliza Park SRS...
Eliza Park SRS has a service care-coordination agreement with Peninsula Health mornington, ensuring a suitable integrated service for supported accommodation. This provides clients who are placed by hospitals, social workers, case managers or any other community service with a continuity of care throughout the transitional phase and ongoing support in identifying and meeting the needs of the individual. The M.I. Health program under Peninsula Health Community Health Mornington provides Eliza Park SRS with a variety of services with those who register. As well as a DIV1 nurse who comes onsite with the visiting G.P. M.I. Health provides ongoing one on one support to residents with follow up on medical appointments, assistance with transport, locating and linking into appropriate services as well as funded services such as podiarty, hearing, optical and dental.
